Registered Nurse (RN) Case Manager – Home Care

  • Field-based position serving patients in their homes in the southeast section of the Twin Cities Metro area, with office support based in Roseville, MN

What you’ll do

As an RN Case Manager, you will provide comprehensive, holistic home care services, including:

  • Perform thorough head-to-toe assessments and ongoing evaluation of patient status
  • Provide care coordination, disease management, and medication management
  • Deliver skilled wound care, catheter care/management, and ostomy education
  • Promote infection prevention and conduct home safety assessments
  • Visit in-home patients to evaluate the quality of care and review clinical documentation
  • Participate in ongoing interdisciplinary assessments and case conferences
  • Provide services ordered by the physician in accordance with the plan of care
  • Counsel and educate patients and families to help them understand and manage their health needs
  • Complete timely, accurate, and detailed clinical notes, reflecting patient progress and changes in condition
  • Communicate effectively with the interdisciplinary healthcare team to coordinate care and plan for discharge or transitions

What we’re looking for

  • Graduate of an accredited school of nursing
  • Current, active Registered Nurse (RN) license in the State of Minnesota
  • Ability to travel within the service area with reliable transportation, valid driver’s license, and proof of auto insurance
  • CPR certification (required)
  • Two (2) years of current nursing experience preferred
  • One (1) year of case management experience preferred
  • Preferred background in community health and a broad range of medical-surgical experience
  • Strong communication, critical thinking, and organizational skills, with a passion for home care and patient advocacy
