PM Registered Nurse (RN) – Monday–Friday, 2pm–10pm

Position Summary

We are seeking a dedicated full‑time PM Registered Nurse (RN) to join our evening team, working Monday through Friday from 2pm–10pm. In this key role, you will coordinate and deliver high‑quality nursing care to long‑term care and rehabilitation residents, support CNAs and other team members, and help ensure a smooth and safe transition into the night shift. Once fully trained, you will participate in our on‑call rotation.


Schedule and Pay

  • Status: Full‑time, PM shift
  • Hours: Monday–Friday, 2pm–10pm
  • On‑call: Included after training
  • Pay range: $37–$42 per hour (competitive, based on experience and other factors)

Key Responsibilities

  • Provide safe, accurate medication administration in accordance with physician orders and facility policies.
  • Perform ongoing assessments and evaluations of long‑term care and rehab residents, recognizing changes in condition and responding promptly.
  • Implement and apply treatments and interventions to meet individual resident needs, including wound care and restorative care as ordered.
  • Observe residents closely and document assessments, interventions, responses, and care in a timely, thorough, and accurate manner.
  • Collaborate with CNAs, therapy, providers, and other interdisciplinary team members to support holistic, person‑centered care plans.
  • Interact with residents and families in a patient, respectful, and compassionate manner—regardless of a resident’s level of cooperation or cognition.
  • Follow all safety, infection prevention, and regulatory guidelines to protect residents, staff, and visitors.
  • Support a positive, team‑oriented environment by communicating clearly, helping coworkers, and modeling Greentree’s C.A.P.L.I.C.O. values.
  • Maintain reliable, consistent attendance and be willing to participate in the on‑call rotation once trained.

Qualifications

  • Current, unencumbered Registered Nurse (RN) license in the State of Wisconsin.
  • Previous experience in long‑term care, skilled nursing, or post‑acute rehab preferred; new RNs with strong clinical skills and a passion for geriatrics are encouraged to apply.
  • Strong assessment, critical‑thinking, and clinical decision‑making skills.
  • Ability to multi‑task and prioritize in a busy PM shift environment.
  • Excellent communication and documentation skills, with comfort using electronic health records.
  • Compassionate, patient, and professional approach to caring for older adults and their families.

COVID‑19 Vaccination Requirement

In accordance with Centers for Medicare & Medicaid Services (CMS) requirements, all facility employees and contractors must be fully vaccinated against COVID‑19 or have an approved medical or religious exemption.
